POSITION SUMMARY

With a focus on children, youth, and families, provides psychiatric assessments, medication monitoring, & continuity of care. Responsible for performing basic screening, diagnostic treatment, rehabilitative and supportive services as a member of a primary and psychiatric health care team for outpatient clients using a client-directed outcome informed approach. This may include direct supervision as well as review and revision of all applicable procedures and their execution relating to the daily medically related activities. Collaborates with Medical Staff in delivering program specific services. Works as an integral part of the clinician staff by participating in Medical Staff and PA/NP meetings. Provide continuous and comprehensive services necessary for the promotion of optimal mental health, prevention and treatment of psychiatric disorders and health maintenance to children and adolescents, ages 12 – 20. This includes the assessment, diagnosis, and management of mental health problems and psychiatric disorders. The PA/NP works collaboratively with other on-site medical and mental health providers as well as with community PCPs and other allied mental health professionals.

ESSENTIAL FUNCTIONS

 

Patient Care Management/Documentation

  1. Work collaboratively with a physician and/or mental health provider of record, (if one exists), to provide comprehensive adolescent mental health services.
  2. Through patient evaluation, determine a differential diagnosis and plan for intervention.
  3. Adhere to standards via identified text and/or via office practice standard protocols.
  4. Maintain a customer-oriented approach to meeting patient needs in an expedient manner, involving the family in the plan of care as appropriate.
  5. Maintain confidentiality and patient privacy.
  6. Document care provided; meeting standards for quarterly chart reviews conducted by an independent 3rd part contractor.
  7. Take appropriate history and physical examination of patients.
  8. Order pertinent diagnostics and implement appropriate therapy according to scope of practice guidelines.
  9. Collaborate with physicians, psychologists, social workers, other psychiatric nurses, or other allied health professionals to discuss treatment plans and progress; review and evaluate treatment procedures and outcomes of other psychiatrists and licensed medical professionals.
  10. Prescribe, direct, and administer psychotherapeutic treatments or medications to treat mental, emotional, or behavioral disorders.
  11. Conduct psychiatric evaluations and ongoing medication management with general knowledge in the areas of Historical Trauma and Trauma-informed Care, domestic violence dynamics, pain management, addictive behavior, and development issues.
  12. Maintains compliance with all aspects of regulatory and funding requirements including OSHA, CLIA and Title 22, etc.
  13. Participate in multidisciplinary Team (MDT) conferencing to coordinate clinic treatment plans with internal and external resources.
  14. Provide psychiatric care within the scope of his/her licensure and in accordance with community standards of care.
  15. Serve on committees to promote and maintain community mental health services and delivery systems.
  16. Participate, as needed, in review and analysis of data including, but not limited to, quality improvement adverse clinical incidents, population demographics and morbidity’s clinical decision-making and consultation with clinicians to ensure ongoing behavioral health into primary care.
  17. Observes and executes medical protocols, policies, and procedures in compliance with AAAHC standards.

 

Consultation and Referral

  1. Obtain supervising physician's consultation when problems presented are outside scope of practice or when unfamiliar with a particular aspect of the patient's care according to the PA/NP Standardized Procedures.
  2. Consultation with Director, Medical Services or attending primary care physician is required if the NP/PA wishes to prescribe more than two (2) controlled substances from the same class of medication.
  3. Demonstrate competent knowledge in delivering health care to patients through the peer review processes including chart review, by interactions with physician preceptors and by working collaboratively with support staff and all staff of SSTHP.
  4. Become an integral part of the clinical staff through participation in Medical Staff meetings, PA/NP meetings, and other departments of SSHWC.
  5. Utilize teaching learning theory with patients and their families to influence patient outcomes.
  6. Identify learning needs of various populations and contribute to the development of educational programs to meet those needs.
  7. Identify system specific problems, which require evaluation and/or change; work with appropriate staff to resolve these issues by participating in performance improvement processes.
  8. Takes an active role in contributing to the improvement of office functions.
  9. Willingly perform other duties within the scope of practice as assigned.
  10. Responsible for maintaining current licenses to practice medicine. Copies are to be given to Human Resources, Executive Director, and Director of Behavioral Health.
  11. Responsible for providing proof of earned CME hours annually to Human Resources and Executive Director and Program Director.
  12. Communicate effectively with the patients while involved with their health care, including providing appropriate patient education while answering questions and in history taking.
  13. Document succinct and comprehensive case records, clinical charts in 48 hours and consistently in accordance with agency policies, ethical guidelines, and legal regulatory requirements.

 

MINIMUM QUALIFICATIONS

 

Education & Experience:

  1. Minimum Education: Completed a nationally accredited Physician Assistant or Nurse Practitioner Master’s degree prepared Program. Current license and be in good standing with the California Licensing Board or current PA certification through the National Commission on Certification of Physician Assistants, Inc., or NP certification through the American Nurses’ Association.
  2. Minimum Experience: 3 or more years in an outpatient setting, with children’s mental health programs or community-based practices.
  3. Preferred Experience: 7+ years in a mid to large size integrated healthcare delivery system including a focus on significant experience treating children.

Knowledge, Skills & Abilities:

  1. Strong supervisory, leadership, interpersonal and organizational skills.
  2. Ability to establish and maintain effective peer to peer relationships with coworkers across multiple health departments.
  3. Ability to work with people of all social and ethnic backgrounds and to resolve conflicts and facilitate consensus.
  4. Willingness to be part of the “bigger” picture, be proactive and function in a rapidly changing healthcare environment.
  5. Knowledge of HIPAA Law, Privacy Acts, and ability to maintain confidentiality of records.
  6. Knowledge of current therapies and ability to involve the supervising physician when appropriate.
  7. Previous outpatient clinic experience working with indigent population preferred.

Other Requirements:

  1. Must be able to successfully complete a pre-employment drug and background screening.
  2. Must have a valid California driver’s license.
  3. Must possess active unrestricted California State license as a certified psychiatric nurse practitioner.
  4. ANCC Certification as a Mental Health Nurse in California.
  5. Must possess active CPR, ACLS certifications and DEA registration to prescribe.
  6. Must be able to work in the United States
  7. Post job offer: CPR/BLS certification, TB test, and annual flu shot (during flu season) are required as a condition of employment, unless a medical/religious accommodation request is provided. Hepatitis B Vaccine will be offered at no cost to employees, but it is not required.

DRUG SCREEN

Shingle Springs Band of Miwok Indians requires the successful completion of a drug screening of all potential Tribal employees prior to being hired. The Tribe recognizes that the State of California has legalized the use of marijuana. The Tribe is located on federally owned trust land and complies with federal laws. Therefore, marijuana use is prohibited, and all drug tests will include a screening for THC.

 

Shingle Springs Band of Miwok Indians recognizes a zero-tolerance drug and alcohol policy, and employees will be required to sign a Consent to Drug and/or Alcohol Testing. Employees may be selected for alcohol / drug testing upon reasonable suspicion, and upon any/all accidents.
